• Dorzolamide is a sulfonamide and topical carbonic anhydrase II inhibitor.
• It exists as a number of isoenzymes, the most active of which is carbonic anhydrase II ( CA-II ).
• One gene is carbonic anhydrase II ( CAII ), which, when mutated, causes osteopetrosis with renal tubular acidosis ( type 3 ) { Sly, 1983 }.
• Inhibition of carbonic anhydrase II in the ciliary processes of the eye decreases aqueous humor secretion, presumably by slowing the formation of bicarbonate ions with subsequent reduction in sodium and fluid transport.
• Immobilization by limb casting does not seem to reduce the relative proportions of muscle-specific proteins, such as carbonic anhydrase II and myoglobin, over that predicted by the overall decrease in muscle protein synthesis.
• Hereditary  Proximal or  Type 2 RTA may be caused by mutation of the SLC4A4 gene and yet a further  Type 3 RTA with combined features of proximal and distal RTA is due to mutation in the CA2 gene for carbonic anhydrase II . These developments in the molecular biology of the kidney may be traced back in large part, though not exclusively, to Wrong s original 1959 paper.
